We are Seeking

We are seeking an experienced Aged Care Services Coordinator to coordinate the day-to-day delivery of Spectrum’s Commonwealth Home Support Programme, Care Finder Program and Support for Carers Program. You will oversee client intake, assessment, service planning and referrals while supporting program staff to deliver safe, person-centred and culturally responsive services. This role will also monitor service quality, compliance, program targets and client outcomes.

About you

  • A relevant tertiary or diploma qualification in aged care, healthcare, community services or a related discipline.
  • Demonstrated experience coordinating services within the aged care or community services sector.
  • Sound knowledge of the new Aged Care Strengthened Standards, aged care programs, client assessment, care planning, referral pathways and person-centred practice.
  • Experience supporting or supervising staff and coordinating workloads in a busy service environment.
  • Strong communication, stakeholder-management, organisational and record-keeping skills, with a commitment to culturally responsive service delivery.
