We are seeking a Care Partner – Client Intake and Assessment who is passionate about supporting older people to access quality, person-centred care services. This role is responsible for conducting intake assessments and home visits, supporting aged care assessments, coordinating services, and managing client care plans to ensure services are delivered in line with client needs, preferences, dignity, and rights. The position also plays an important role in clinical governance, quality improvement, risk assessment, and supporting the transition to the Support at Home program, while promoting a positive client experience and maintaining strong stakeholder relationships.
About you
We want to hear from you if you have:
- Experience in aged care, care management, intake and assessment, or community services.
- Strong understanding of person-centred care, client rights, and service coordination.
- Excellent communication, relationship-building, and stakeholder engagement skills.
- The ability to manage competing priorities, documentation, risk assessments, and client caseloads effectively.
- A proactive and compassionate approach, with commitment to quality improvement, clinical governance, and client wellbeing.
